EDUCATION and EXPERIENCE:
- High School Diploma or GED, is required;
- Minimum of one to two years of office and job-related experience, is required;
- Must be able to read, write and speak in both Spanish and English languages;
- Must have two to three years of Microsoft Office experience;

JOB TITLE: ACQUIRED BRAIN INJURY CASE MANAGER
JOB SUMMARY:
Under the supervision of the Program Coordinator oversee and determine eligibility for program participant’s services by conducting assessments; develop and implement care plans to maximize participant choices and independence.
