Case Management Services
Home Based Services
Home Based Services permit families to have a primary case manager to provide services in the homes in order to achieve permanence for children or adults, while maintaining and strengthening the family integrity. Individuals are able to still receive services outside of the home, but the primary case manager conducts home visits to ensure they are receiving sufficient services. Families may request in home support for personal support for the individual with regard to the achievement of goals.
