JOB SUMMARY:
Uses standard case file management procedures to screen and process prospective adult program participants and utilizes information for vocational and educational planning. Additionally, provide leadership, direction and implementation within the State of Illinois Latino Outreach Grant proposal goals and objectives.
JOB FUNCTIONS: (include but are not limited to the following)
- Review and coordinate all potential cases for new participants a timely manner as required by accreditation bodies and funding sources.
- Conducts comprehensive interviews with prospective participants in order to gather necessary information.
- Maintains and reviews case files, including referrals and follow-ups.
- Conducts monthly outreach, recruitment and referral activities through phone calls, mail distributions and public presentations to potential referral sources and funding entities.
- Provides monthly and quarterly reports, completes required paper work and analysis regarding adult program activities, according to the standards of the appropriate funding agency.
- Ensure the successful implementation of all activities required by the outreach grant(s) including: community activities, community outreach, employment workshops, classes, and support groups.
- Compile all reports required by the guidelines of the State of Illinois with regard to the grant;
- Work with all staff associated with the program to ensure that programs are implemented in accordance with grant guidelines;
- Design, implement and track pre and post satisfaction surveys for all different activities;
- Chairs the intake review committee.
- Other duties as assigned.
QUALIFICATIONS AND REQUIREMENTS:
- Associates degree from an accredited college required;
- At least 3 years of experience working with the Intellectual and Developmental Disability (IDD) population preferred;
- Passion and commitment working with adult with intellectual and developmental disabilities;
- Fluent verbal and written English and Spanish communication preferred;
- Successful completion of First Aid/CPR certification required within four months of hire or current certification;
- Completion of a DSP certification training program preferred but not required (training may be provided);
- Must pass a reading comprehension test and pre-employment background check screening requirements;
- Current, valid Illinois driver’s license and proof of insurance required;
- Reliable means of transportation required.

WAGE/SALARY RANGE:
- $19.50 – $20.00 / hour
- Based on experience and education
WORK LOCATION:

- Flexibility to travel to other worksites/work assignments required.

- 40 hours per week ( 1 hour meal break)
- Evenings as needed
- Weekends as needed
- Overtime as needed
